Our qualified and registered nurses deliver personalised care plans that support your health needs while promoting independence, dignity, and wellbeing.

We assist participants with a wide range of clinical and complex care needs, including medication management, wound care, continence support, diabetes care, catheter and stoma care, PEG feeding, and ongoing health monitoring. Our nurses work closely with participants, families, carers, and allied health professionals to ensure consistent, high-quality care.

Eligibility for NDIS Community Nursing Services

You may be eligible for NDIS Community Nursing Services if you are an NDIS participant and have nursing or clinical support needs related to your disability. These supports must be considered reasonable and necessary under your NDIS plan.

• Are approved NDIS participants with an active NDIS plan
• Have a disability-related health condition requiring ongoing or intermittent nursing care
• Have Community Nursing or High-Intensity Supports funded in their NDIS plan
• Require clinical support such as medication management, wound care, continence care, diabetes management, catheter or stoma care, PEG feeding, or complex health monitoring
• Need nursing support to maintain health, prevent hospitalisation, or safely live in the community
